FORTIFY - For Young Adult Cancer Survivors

Thank you for your interest in participating in our research. You are being asked to participate in this study because you are between the ages of 18-39, have a history of cancer, and have completed cancer treatment between six months and four years ago. The purpose of this research study is to find out more about life after cancer, specifically to understand the impact of cancer on thoughts and feelings about the future among young adult survivors.

Participation in this study will involve:

- An online questionnaire, which is estimated to take 15-30 minutes

- An online video-recorded interview, which will last approximately 60 minutes

- Compensation in the form of an Amazon Gift Card following completion of the online survey and interview.

- Permission to audio and video record the interview session. You will have the right to erase the video and audio recording taken as part of this research.
